  • Admin Requirements  Submit a completed application form located at Submit a $60.00 nonrefundable application fee payable to Chamberlain College of Nursing. Request official transcript documenting graduation from a state approved high school or successful completion of the General Education Development Examination (GED). Request official transcripts from all post-secondary schools attended to be sent directly to Chamberlain College of Nursing. Submit a photocopy of unrestricted nursing license (if applicable). Request a copy of ACT or SAT scores to be sent directly to the College; Chamberlain College of Nursing’s school code for the ACT is 2293 and for the SAT is 3139. Submit the statement of eligibility, properly signed and dated, concerning the State Board of Nursing Examination (NCLEX), found on the back of the application. International students must submit an official transcript evaluation from a NACES approved evaluating agency. Applicants whose native language is not English must complete the Test of English as a Foreign Language (TOEFL). Admission documents may be mailed or faxed to the address indicated on the application.
